Multiple small failures really adding up.

A fairly interesting article over at PeterMe on Jumping through hoops. This is how that with the greater number of steps it takes for someone to do something, the less successful if will be. Which, when you think about it, makes perfect sense.

Complex systems fail because of the combination of multiple small failures, each individually insifficient to cause an accident. These failures are latent in the system and their pattern changes over time.

The latency of the small failures seems an interesting idea. My first, gut, reaction to this (without reading David Woods stuff on human error, is that this is the sort of thing Defensive Design trys to combat. But that's just my initial gut reaction. An interesting thought though.

How simple should things be? As simple as possible, duh. But what is the trade-off, and where? How should we manage this complexity that is going to creep in? (via Digital Web)
