This form uses JavaScript unobtrusively to provide a great deal of additional usability features.

Hiding Non-required Fields. This will shorten forms for users that desire it.
Immediate Form Validation. Fields marked as required will be lightly proofed once the user moves off the field.
Add new form fields dynmaically. Try adding mulitple address rows. Then remove them.
Automatic Formatting. Try putting in any 10 digits in any format into the Phone Number Field.
Yellow Fade. This provide a clear indication on the page of what has changed and what you need to focus on.
On Leave Warning. If you start the form, and try and navigate away from the page without submitting it you'll get a reminder.
Custom Check Boxes.
Preselecting. Click into the text area and you'll see that the entire text is selected for easy deletion or modification.
